Name Changed When 3 Months Old

Hi, My question is about your number 2 question. I was put up for adoption and I wasn't adopted until I was 3 months old. From what I know, my birth name was Carol, named after my birth mom, Carol. On the papers I have, it states Baby Carol, that is what I was called for the first 3 months of my life and then when I was adopted my name was changed to [name removed].
