Name and Birth Date to Use
Question:
My question has 2 parts: 1. Should we use actual date of birth OR official (documented) date of birth while using your calculators, if both are different? 2. Should we use birth name or the current name, if name was changed later?

For the birth date, use the actual date of birth, the date of the physical event.
For the name, it depends. If it is for a specialized reading, then provide the name it asks for (first name, family name, newly assigned name, nick name, …). If it is for a personal numerology chart, then generally the name as first officially recorded at birth is the correct one to provide.
If a person has had a new name for many years and strongly identifies themself with the new name, then both names have an influence on the person's characteristics. In that case, do a second reading with the current name. The two can then be read in conjunction, any conflicts resolved according to what is the person's actual current experience.
