For traditional Pythagorean numerology readings that ask for the name at birth or ask for the full name, provide what was recorded with the birth certificate. If "the 4th" is part of the name on the birth certificate, then include it for calculation with the name, the same calculations as would be done for any name.
If "the 4th" is not part of the name on the birth certificate, don't include it with name calculations.
Other numerology practices may have different expectations.
