Social, Creative: 3

Active: 1 and 5

No lacking or limitation: 0 (but the number 0 won't be found in a name)

Perhaps you'll be able to fit most of your meaning requirements in a name. If not, a compromise may be necessary. For example, you already have a 3 birth date number, so perhaps 3 can be dispensed with for the name.
